Clinical definition of gliomas is constrained by the invasiveness of tissue biopsies and the restrictive function of the blood-brain barrier. Furthermore, defining minimal residual disease remains challenging due to the diagnostic ambiguity in differentiating tumor progression from treatment-induced pseudoprogression via conventional neuroimaging. Our approach introduces an integrated multimodal liquid biopsy framework designed to overcome these limitations. By combining circulating tumor cells cytology with proteomic and metabolomic profiling of nasal microenvironment analytes, we developed a noninvasive diagnostic paradigm shift for intracranial neoplasms with comparable sensitivity vs gold-standard tissue biopsy. This strategy significantly reduces procedural morbidity, optimizes decision-making, and enhances longitudinal patient monitoring. Subsequent integration of Digital Cytology and Machine Learning marks the transition toward proactive to actionable clinical intelligence, transforming raw data into a personalized clinical roadmap.
