ImpactCare

Incorrect use of medications is one of the greatest risks for multi-morbid elderly patients. With an increased number of medications, the risk of drug-related problems increases, which is estimated to be behind one in five hospital admissions of elderly patients. The risks of drug problems are particularly high during transitions in care. ImpactCare: a strengthened chain of drug information across care boundaries, is a project aimed at improving communication about medications at discharge from hospitals to increase patient safety and reduce unnecessary healthcare injuries. The project is a collaboration between Region Uppsala, Region Västmanland, participants from pensioner and relatives' organizations, as well as the Department of Pharmacy and the Department of Public Health and Caring Sciences at Uppsala University.
