Bo Nilsson – Det intravaskulära medfödda immunsystemet och tromboinflammation
Vår forskning fokuserar på det intravaskulära medfödda immunsystemet, med målet att utveckla kombinerade diagnostiska och behandlingsstrategier som modifierar detta system i sin helhet.
Det intravaskulära medfödda immunsystemet, IIIS (för engelskans Intravascular Innate Immune System), består av blodets kaskadsystem tillsammans med cirkulerande celler och celler i blodkärlsväggarna. Det ligger bakom ett brett spektrum av reaktioner i samband med blodpropp och inflammation, så kallad tromboinflammation, både under sjukdom och vid sjukdomsbehandling.
IIIS är ett gammalt försvarssystem
IIIS har en evolutionärt gammal fysiologisk roll för att hantera avfall och laga skador. Det sker genom att eliminera sjukdomsalstrande mikroorganismer, avlägsna döda celler och främmande partiklar samt att förbereda för vävnadsreperation. I de flesta djurarter som saknar ett adaptivt immunsystem utgör IIIS det huvudsakliga försvaret mot infektioner och vävnadsskador.
Komponenter i IIIS
Kärnan i IIIS består av blodets kaskadsystem som i sin tur utgörs av komplement-, koagulations- och fibrinolyssystemen samt det så kallade kallikrein–kininsystemet. Dessutom ingår ett antal igenkänningsmolekyler, immuncellerna granulocyter och monocyter samt blodplättar. Genom en väl utbyggd kommunikation kan aktivering av ett system snabbt spridas till de andra, vilket leder till en koordinerad tromboinflammatorisk respons.
Tromboinflammation
Tromboinflammation är den tätt sammankopplade kombinationen av blodproppsbildning och inflammation som normalt bidrar till att laga kärlskador, rekrytera vita blodkroppar, avlägsna skadad vävnad och stödja läkning. Men om det uppstår alltför en kraftig eller felreglerad tromboinflammation kan det ge vävnadsskador och försämrad organfunktion. Eftersom IIIS främst kan skilja mellan ”själv” och ”icke-själv” på artnivå kan det även attackera egna celler som förändrats under infektioner, vid syrebrist eller av mekanisk stress.
Experimentella modeller
Historiskt sett har forskningen på ovanstående system varit fragmenterad och främst fokuserat på delar av dem. Det har försvårat analyser av verkliga interaktioner.
Vi har utvecklat experimentellt modeller med ytor som behandlats för att bli anti-trombotiska och som tillåter försök där helblod kan användas med minimal artificiell aktivering. De har använts för att visa på tidigare okända interaktioner inom IIIS vilket har förflyttat fokus från isolerade kaskadreaktioner till ett enhetligt intravaskulärt medfött immunologiskt nätverk.
IIIS som ett integrerat system
Vårt övergripande mål är att studera IIIS som ett integrerat system och att utveckla en kombination av strategier för diagnostik och behandling som kan modifiera hela IIIS. Detta kan illustreras av tillstånd som covid-19, blodcirkulation utanför kroppen (t.ex. hjärtlungmaskin) och reperfusionsskada efter syrebrist, där samma grundläggande mekanismer driver väldigt olika kliniska syndrom.
