Infertilitet är vanligt förekommande hos kvinnor i åldrarna 15 till 44 år och mer än 12% av alla par i reproduktiv ålder är ofrivilligt barnlösa. Paren kan ofta få hjälp via assisterad befruktning, sk in vitro fertilisering (IVF). I de fall som paret saknar funktionsdugliga äggceller kan behandling genomföras med könsceller från en donator. Sedan 2003 är IVF med donerade ägg tillåten i Sverige.
Den ursprungliga indikationen för äggdonation var ovariell svikt. På senare år har indikationer baserat på hög ålder, nedsatt ovarial reserv, sekundär infertilitet beroende på tex behandling av malignitet tidigare i livet, upprepade misslyckade IVF-försök och risk för nedärvning av genetiskt dominant sjukdom tillkommit. Ett möjligt samband med ökad risk för graviditetskomplikationer vid äggdonation föreligger. De graviditeter som lyckas där ett ägg donerats skiljer sig från en normal spontangraviditet på många sätt. Embryot som återförs bär på ett fetalt genom som är helt främmande för kvinnan och komplexa immunologiska interaktioner mellan kvinnan och fostret måste hanteras. Vi ämnar därför genom en registerstudie att studera om et finns medicinska skillnader när det gäller graviditetskomplikationer, neonatala och perinatala utfall hos kvinnor som genomgår äggdonationsbaserat IVF jämfört med konventionell IVF- och/eller spontan- gravida kvinnor.
